Responsibilities

• Advocate for patients’ needs and rights within healthcare and social services systems.
• Conduct comprehensive assessments to determine patient needs, goals, and eligibility for services.
• Coordinate services and referrals to healthcare providers and community resources.
• Deliver termination of benefits letters and ensure patient understanding of rights and obligations.
• Develop individualized care plans in collaboration with patients and professionals.
• Maintain accurate records of patient interactions, assessments, and plans.
• Provide information about services, resources, and strategies to manage conditions.
• Monitor patient progress and adjust discharge plans as needed.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
