Job summary

Select Specialty Hospital is seeking a Case Manager for a full-time, weekday schedule in our Critical Illness Recovery Hospital. The role requires licensure in nursing, RT, or Social Work per state guidelines and prefers candidates with discharge planning experience.

The Case Manager will handle utilization reviews, discharge planning, treatment and financial management while coordinating with payors and a multi-disciplinary team to ensure high-quality post-ICU care for chronically ill patients.

Qualifications

• Requires licensure in nursing, RT, or Social Work per state guidelines.
• Inpatient discharge planning experience preferred.

Responsibilities

• Develops patient-specific, safe, timely discharge plans.
• Performs utilization criteria reviews and documents outcomes.
• Coordinates with payors to maximize reimbursement and manages resources.
• Adheres to Utilization Management policies and procedures.
• Coordinates UR with Quality Management to ensure licensure and accreditation.
• Manages department fiscal responsibilities and insurance verifications.
• Leads multi-disciplinary meetings with physicians, nurses, RTs and therapists.
