Job summary

Hospital Sisters Health System (HSHS) seeks a Care Manager-RN to join our Case Management team at St. Elizabeth's Hospital in O'Fallon, IL. The role focuses on coordinating care, discharge planning, and ensuring efficient patient progression through the continuum of care.

Requirements include an RN license in IL or WI, an Associate degree or Diploma in Nursing, and at least 3 years of clinical experience. Bachelor's degree and case management certification are preferred.

Qualifications

• Associates degree or Diploma in Nursing is required.
• Bachelor's in Nursing is preferred.
• 3 years of clinical experience is required.
• Prior case management experience is preferred.
• Case Management certification (ACM CCM RN-BC) is preferred.

Responsibilities

• Collaborate with patient/family, social workers, physicians, and payers to coordinate care.
• Manage discharge planning to optimize patient flow and transitions.
• Assess and plan to ensure care in the most clinically appropriate setting.
• Support regulatory compliance and core competencies within the organization.
