Qualifications

• Associate degree in nursing or diploma from an accredited program.
• 3 years of recent clinical experience with patient care management including education, discharge planning, and utilization management.
• Virginia RN licensure required; BLS-HCP within 6 months of hire.

Responsibilities

• Provide case management for assigned patient populations to achieve optimal clinical and resource outcomes.
• Perform admission assessments and ongoing needs assessments to support care planning and discharge planning.
• Coordinate transitions of care, including post-acute services and home health or hospice.
• Collaborate with physicians, nursing, social work and utilization review to align the plan of care and discharge plan.
• Document patient condition, interventions, and resource utilization per standards; support performance improvement initiatives.
