Job summary

The RN Case Manager at Carilion Franklin Memorial Hospital in Rocky Mount, VA coordinates care for assigned patient populations, drives discharge planning, and supports utilization management. Qualifications

• RN with hospital experience and case management responsibilities.
• Bachelor's degree in Nursing required; Virginia RN licensure is required.
• Experience coordinating discharge planning and post-acute care.

Responsibilities

• Coordinate care and services for the case-managed population.
• Perform face-to-face assessments and document plan of care.
• Collaborate with UR, social work, and care teams to optimize outcomes.
• Facilitate referrals to home health, hospice, and DME as needed.
• Ensure regulatory compliance and accurate status communication.
