Job summary

Carilion Clinic is seeking an RN Case Manager in Roanoke, VA, to provide comprehensive case management services. The role involves coordinating care across the health care continuum, assessing patient needs, and facilitating referrals to post-acute services.

Strong collaboration with UR and social work teams is essential to optimize outcomes. The position requires a Virginia RN license, a Bachelor's degree in Nursing, and BLS-HCP certification within 6 months of hire.

Qualifications

• Registered Nurse with Virginia licensure.
• Bachelor's degree in Nursing required; 5 years hospital RN experience may substitute.
• AHA BLS-HCP certification within 6 months of hire.

Responsibilities

• Provides case management for assigned patient populations across the care continuum.
• Collaborates with Utilization Review Nurse to optimize patient status and care transitions.
• Uses InterQual to determine medical necessity and coordinate referrals.
• Documents plan of care, discharge planning, and CMS-compliant records.
• Coordinates post-acute referrals (home health, hospice, DME) and coordinates with social work.
• Assesses patient needs, communicates with patients/families about discharge and financial implications.
• Participates in Interdisciplinary Team Meetings and performance improvement initiatives.
