You will oversee case management for assigned patient populations, coordinate discharge planning, and collaborate with social work and utilization review teams to optimize care.

Requirements include Virginia RN licensure, a Bachelor's degree, and 3 years of hospital case management experience; BLS; InterQual familiarity; strong communication and teamwork skills.

Qualifikationen

• Registered Nurse license in Virginia.
• Bachelor's degree required; 5 years RN hospital experience may substitute.
• Three years of recent clinical care experience including discharge planning and utilization management; supervisory/leadership preferred.
• AHA BLS-HCP within 6 months of hire.
• Knowledge of InterQual and discharge planning; strong communication skills.

Aufgaben

• Collaborates with Utilization Review Nurse and social work staff.
• Coordinates care and services within the case managed population; conducts face-to-face assessments when appropriate.
• Documents in the medical record to reflect plan of care and CMS discharge regulations.
• Manages referrals to post-acute services (home health, hospice, DME) and coordinates transitions of care.
• Participates in interdisciplinary team meetings and performance improvement initiatives.
