Job summary

Carilion Clinic is seeking a Weekend RN Case Manager to lead case management for assigned patient populations in a full-time weekend role. The role emphasizes interdisciplinary collaboration, discharge planning, and efficient care transitions within the Roanoke facility.

The position requires an RN license in Virginia, a Bachelor's degree, and 3+ years of clinical experience. Sign-on bonus and relocation assistance are available, with a weekend 12-hour shift schedule (Thu-Sat, 7am-7pm).

Qualifications

• Registered Nurse with current Virginia licensure.
• Bachelor's degree required; RN experience may substitute in some cases.
• Three years of clinical health care experience with patient management duties.
• AHA BLS-HCP certification within 6 months of hire.

Responsibilities

• Perform patient needs assessments on admission and during care.
• Coordinate referrals for post-acute services and DME, HH, hospice.
• Collaborate with Utilization Review Nurse and SW partner throughout the day.
• Communicate discharge plans and financial implications with patients/families.
• Maintain accurate documentation reflecting plan of care and interventions.
