Job summary

Carilion Clinic seeks an RN Case Manager for weekend shifts at Carilion Franklin Memorial Hospital in Rocky Mount, VA. The role centers on coordinating care, performing needs assessments, and guiding patients through the health care continuum to optimize outcomes.

The position requires VA RN licensure, a BSN (or 5 years’ hospital experience in lieu), 3 years of clinical health care experience, and BLS within 6 months. Strong communication is essential.

Qualifications

• RN with Bachelor’s degree required; 5 years hospital experience may substitute for degree.
• Three years of clinical experience in patient care management including discharge planning and education.
• Current VA Registered Nurse license; BLS required within 6 months of hire.

Responsibilities

• Coordinate care and services within the case managed population.
• Perform face-to-face patient assessments and re-assess as needed.
• Collaborate with Utilization Review and Social Work to optimize discharge plans.
• Communicate with patients and families regarding financial implications of discharge plans.
• Maintain timely and accurate documentation reflecting care plan and interventions.
• Participate in Interdisciplinary Team meetings and performance improvement initiatives.
• Coordinate referrals to post-acute services and assist with prior authorizations.
