Bon Secours Mercy Health in Richmond, VA is seeking an RN Care Manager or Social Worker for weekend coverage (Saturday and Sunday) to coordinate patient care across transitions.

You will assess needs, collaborate with the care team, document plans and ensure resources are used efficiently. The role emphasizes compassionate, patient-centered care within a faith-based system and offers competitive benefits.

Qualifications

• 1 year of experience in a clinical setting.
• 3 years of experience in an acute care clinical setting (preferred).
• Ambulatory or post-acute, care coordination experience (preferred).
• BLS Basic Life Support – American Heart Association (required).
• RN License in the state of work or compact; or Social Worker with BSW.
• ACCM/CCM/ANCC certifications preferred.

Responsibilities

• Identifies and prioritizes patients in need of care management services using a holistic approach.
• Completes documentation and handovers at level-of-care changes and care transitions.
• Documents plan of care addressing post-hospital needs with patient/family participation.
• Follows standardized practices for Advance Care Planning, LOS management and readmission prevention.
• Supports denial prevention by addressing barriers to progression of care.
• Promotes proactive care, timely testing/treatment, and appropriate resource use.
