Qualifications

Responsibilities

• Coordinate care for patients to support safe transitions across the continuum.
• Identify and assess needs, plan and evaluate options with the care team.
• Screen and assess individuals for active case management and promote resources.
• Follow standardized practices for advance care planning and discharge rounds.
• Support timely testing and treatment and address barriers to progression of care.
• Ensure appropriate use of resources and communicate plans to the team.
