Job summary

Christus Hospice and Palliative Care is seeking an Afterhours Registered Nurse to provide comprehensive hospice care for patients and families after regular hours, including nights and weekends. The role requires on-call work from Friday 4:30pm to Monday 8:00am, with duties spanning triage, assessments, and afterhours visits.

Responsibilities include initiating timely evaluations, managing crisis calls, and documenting care while adhering to patient-centered protocols.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond to crisis calls from patients and families after hours with urgency.
• Make the initial nursing evaluation within 48 hours of referral, as part of a patient visit.
• Conduct afterhours visits including admissions, death visits, recertification visits, and routine visits.
• Identify physical, psychosocial, emotional, and environmental needs and reassess at least every 15 days.
• Document problems, goals, interventions, and patient/family responses to hospice care.
