Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license and access to a reliable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience (preferred).

Responsibilities

• Triage patient needs over the phone until an in-person nurse visit is available.
• Respond promptly to crisis calls from patients and families after hours.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours admissions, death visits, recertifications and routine visits.
• Document problems, goals, interventions, and patient/family responses to hospice care.
• Adapt care to diverse home environments and reassess needs at least every 15 days.
