Job summary

PACS Group is seeking an Activity Assistant to support the Activity Director in delivering a creative and interactive activities program for residents. The role emphasizes adherence to federal, state, local, and corporate standards and aims to meet the physical, mental and psychosocial needs of residents.

You will plan and participate in activities, coordinate calendars, and assist with assessments and discharge planning while maintaining a safe, clean department and facilitating communication

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Facilitate communication between staff, residents, families, support personnel and the public.
• Develop monthly activity calendar and maintain attendance records.
• Assist in discharge planning and activity care plans and resident assessments.
• Arrange transportation for residents for outings, appointments, or discharge.
• Create monthly activity schedule including outings, group activities and in-room activities.
• Encourage participation in hobbies, crafts and reading.
• Provide reading materials in Braille or audio books as appropriate.
• Assist with assessment documentation and quarterly progress notes.
• Keep the Activity Department clean, orderly and secure.
