Job summary

Diablo Valley Post Acute in Concord, CA is seeking a dedicated Physical Therapist Assistant (PTA) for a PRN weekend position to support our rehabilitation team.

You will work under a Physical Therapist to improve residents’ strength, mobility and independence, provide skilled interventions, educate caregivers, and document care while collaborating with an interdisciplinary team in a supportive in-house therapy setting.

Qualifications

• Current California PTA license.
• Graduate of an accredited Physical Therapist Assistant program.
• Skilled nursing or rehabilitation experience preferred.

Responsibilities

• Provide skilled physical therapy interventions under the direction of a licensed Physical Therapist.
• Assist residents in mobility, strength, balance and functional independence.
• Implement individualized treatment plans and activities.
• Monitor resident response and communicate progress to the Physical Therapist.
• Educate residents and caregivers on safe mobility and exercises.
• Collaborate with nursing and the interdisciplinary care team.
• Maintain accurate and timely clinical documentation.
