Job summary

Concord Post Acute is seeking a compassionate Physical Therapist Assistant (PTA) to join our in-house rehabilitation team on a PRN/per-diem basis in Concord, CA. The role offers flexible scheduling and competitive pay, with the opportunity to support residents in their recovery journeys.

The PTA will provide skilled treatments under the supervising therapist, document progress and outcomes, and collaborate with the care team.

Qualifications

• Active California PTA license in good standing.

Responsibilities

• Provide skilled physical therapy treatments under the direction of the supervising Physical Therapist.
• Accurately document daily treatments, progress notes, and patient outcomes.
• Collaborate with nursing staff, therapists, and interdisciplinary team members regarding resident care plans.
• Participate in patient care conferences and ongoing treatment planning.
• Assist in training and supporting Restorative Aides to promote continuity of care.
• Educate patients, families, and caregivers on maintenance programs and safe discharge planning.
• Maintain professional, compassionate communication with residents, families, and staff.
