Diablo Valley Post Acute is seeking a dedicated Physical Therapist Assistant (PTA) for a PRN weekend position in Concord, CA. You will work under a Physical Therapist to support residents’ strength, mobility and functional independence in a post-acute rehab setting.

The role emphasizes compassionate, resident-centered care and collaboration with an interdisciplinary team. Competitive weekend pay and an in-house rehabilitation environment offer a meaningful opportunity in post-acute care.

Qualifications

• Current California PTA license.
• Graduate of an accredited PTA program.
• Skilled nursing or rehabilitation experience preferred.
• Strong communication, organizational and interpersonal skills.

Responsabilités

• Provide skilled physical therapy interventions under the direction of a licensed Physical Therapist.
• Assist residents in improving mobility, strength, balance, coordination, and functional abilities.
• Implement individualized treatment plans and therapeutic activities.
• Monitor resident response to treatment and communicate progress or concerns to the Physical Therapist.
• Educate residents and caregivers on safe mobility, exercises and therapeutic techniques.
• Collaborate with nursing and other members of the interdisciplinary care team.
• Maintain accurate and timely clinical documentation.
• Provide compassionate, resident-centered care focused on meaningful functional outcomes.
