Job summary

Pleasant Hill Post Acute in Pleasant Hill, CA is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. The PTA will work under a supervising Physical Therapist to deliver high-quality rehab services aimed at improving mobility, safety, and independence for residents.

Expected to perform skilled treatments, assist residents toward mobility goals, and maintain compliant documentation.

Qualifications

• Associate degree from an accredited PTA program.
• Current California PTA license.
• Skilled nursing or post-acute rehab experience preferred.
• Strong communication, organization, and teamwork skills.
• Dependable and flexible for PRN/as-needed work.

Responsibilities

• Provide skilled PTA treatments as directed by the supervising Physical Therapist.
• Assist residents to achieve mobility and functional goals.
• Complete timely, accurate clinical documentation.
• Maintain a safe, clean treatment environment with infection control procedures.
• Communicate resident progress and changes with the supervising PT.
• Collaborate with nursing and interdisciplinary team members.
• Participate in Resident Care Conferences and rehab meetings.
• Assist with training Restorative Aides.
• Educate residents and families on treatment plans and safety techniques.
• Recommend durable medical equipment and adaptive equipment as appropriate.
• Provide staff education and in-service training as needed.
• Report equipment concerns and help keep rehab equipment safe and functional.
• Participate in quality assurance and performance improvement activities.
