Job summary

Ridgeway Post Acute in California seeks a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a part-time/PRN basis. Under supervision of a licensed PTA, you will deliver high-quality interventions to help residents regain strength, mobility, and independence.

You will document progress, maintain safety, and collaborate with the interdisciplinary team to support discharge planning and family education.

Qualifications

• Current and unrestricted Physical Therapist Assistant license (PTA)
• Graduate of an accredited PTA program
• Strong communication and interpersonal skills
• Ability to work effectively within a multidisciplinary healthcare team
• CPR certification (preferred if required by state regulations)

Responsibilities

• Provide physical therapy treatments as directed by the supervising Physical Therapist and/or Director of Rehabilitation.
• Document resident treatments, progress notes, and outcomes accurately and timely per facility policies, Medicare guidelines, and regulatory requirements.
• Maintain a safe treatment environment by adhering to infection control, safety, and equipment maintenance standards.
• Participate in care plan meetings, resident care conferences, and rehabilitation team meetings as needed.
• Assist with training and education of Restorative Nursing Aides and other team members.
• Communicate regularly with the supervising Physical Therapist regarding resident progress, treatment goals, and discharge planning.
• Educate residents, families, and caregivers on therapy goals, home exercise programs, safety techniques, and fall prevention.
• Recommend and facilitate procurement of appropriate durable medical equipment (DME).
• Collaborate with physicians, nursing staff, therapists, residents, and families to support optimal outcomes.
• Maintain resident confidentiality and comply with HIPAA and facility policies.
