Job summary

Diablo Valley Post Acute in Concord, CA is seeking a compassionate Physical Therapist (PT) to join our in-house rehabilitation team. The role offers Full-Time, Part-Time, and PRN opportunities, with pay starting at $60/hour DOE.

You'll evaluate and treat patients, collaborate with a multidisciplinary team, and support patients’ independence and quality of life in a fast-paced, patient-centered program available seven days a week. Current California PT license is required.

Qualifications

• Current California Physical Therapist license required.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide evidence-based therapy interventions to improve mobility, strength, balance and function.
• Supervise Physical Therapist Assistants in accordance with California State Practice Act guidelines.
• Collaborate with physicians, nursing staff and interdisciplinary teams to coordinate patient care and discharge planning.
• Recommend durable medical equipment (DME) and educate patients and families on home exercise programs.

What You'll Do

As a Physical Therapist, you'll help patients regain strength, mobility, and confidence by developing personalized rehabilitation programs focused on maximizing independence and improving quality of life.

• Provide evidence-based therapy interventions to improve mobility, strength, balance, and function
• Supervise Physical Therapist Assistants in accordance with California State Practice Act guidelines
• Collaborate with physicians, nursing staff, and interdisciplinary care teams to coordinate patient care and discharge planning
• Recommend durable medical equipment (DME) and educate patients and families on safe mobility and home exercise programs
• Maintain accurate and timely clinical documentation while monitoring patient progress and adjusting treatment plans as needed

Qualifications

• Current California Physical Therapist (PT) license required
