Advocate Health Wake Forest Baptist Outpatient Physical Therapy is seeking a PRN Physical Therapist to support weekend coverage and outpatient therapy teams. You will provide patient-centered care with strong communication and teamwork, while maintaining accurate documentation and complying with department standards.

Ideal candidates hold a BS in Physical Therapy, with MS or DPT preferred, and a current state license plus CPR certification renewed every two years.

Qualifications

• BS degree from an accredited physical therapy school required.
• MS or DPT preferred.
• Licensed to practice Physical Therapy in the applicable state with current licensure; CPR certification required every two years.

Responsibilities

• Provides patient care assessment and treatment.
• Maintains accurate patient documentation and progress notes.
• Ensures compliant documentation of education, charges and ICD-9 codes as appropriate.
• Meets department productivity standards and budget expectations.
