• Licensed to practice Physical Therapy in the state with current licensure.
• CPR certification required every two years.

Responsibilities

• Provides patient care assessment and treatment following Standard of Practice.
• Maintains accurate patient documentation, evaluations, progress notes, discharge notes; documents education, charges and statistics.
