Job summary

Advocate Health in Charlotte, NC is seeking a Physical Therapist to evaluate patients, design and implement comprehensive treatment plans and supervise PTA/ Rehab Tech staff. The role requires a Bachelor's degree in Physical Therapy, active state license and BLS certification.

You will coordinate care, document progress and communicate with physicians and care teams. This position will provide weekend support with flexibility to work Thursday-Saturday or Friday-Sunday at multiple Carolinas

Responsibilities

• Evaluates patients and develops comprehensive treatment plans.
• Gathers data from medical records and patients for accurate documentation.
• Establishes functional goals with patients and provides evidenced-based interventions.
• Documents evaluations, progress notes, and charges in the patient record.
• Communicates progress with physicians, referrals, and the care team.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
