Job summary

Advocate Health is seeking a Physical Therapist to evaluate patients, establish treatment plans, and implement evidence-based rehabilitation to improve function. The role includes coordinating care, documenting progress, and communicating with physicians and staff in a fast-paced clinical setting.

The position requires a Bachelor's degree in Physical Therapy, an active state license, and BLS certification. Weekend coverage is available with flexible scheduling at multiple Carolinas

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Applicable state license for Physical Therapy is required.
• BLS certification required per policy guidelines.

Responsibilities

• Evaluates patients and develops a care plan with modalities, exercises, and education.
• Gathers data from medical records, physician reports, and patient/family through documentation.
• Establishes functional goals with the patient and provides evidenced-based interventions.
• Documents evaluations, tests, progress notes, and discharge information.
• Communicates evaluation results and progress to physicians and other clinical staff; coordinates schedules and discharge planning.
• Supervises PTA's and Rehab Tech/Aide as required by licensure.
