Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) PRN to support weekend staffing at our Gallatin, Tennessee facility. The PT will assess patients, develop treatment plans, and work under the direction of the physician to achieve goals.

The role requires licensure, documentation proficiency, and collaboration with a multidisciplinary team to deliver patient-centered care in a long-term care setting. Evening and weekend shifts are available.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid physical therapy license and state registration.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care/geriatric settings preferred.
• Strong documentation and charting skills.
• Collaborates with multidisciplinary teams to deliver patient-centered care.
• Adheres to confidentiality, HIPAA, and Residents' Rights.

Responsibilities

• Assess referred patients and provide direct rehabilitation care.
• Develop and administer individualized treatment plans.
• Document patient condition, progress, and discharge planning.
• Educate patients and families on physical therapy methods.
• Coordinate with physicians and other disciplines to optimize care.
