Job summary

Gallatin Center for Rehabilitation and Healing seeks a Physical Therapist (PT) to join our facility on a PRN basis, primarily for weekend staffing. The PT will assess patients, develop treatment plans, and document progress under physician direction, while educating families as needed.

Qualified candidates will be licensed in their state, experienced in geriatric care, and capable of supervising PTAs. Professionalism and timely attendance are essential to support high-quality patient care.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Valid PT license and state registration.
• Supervises Physical Therapy Assistants.
• Experience in long-term care or geriatric care preferred.
• Strong documentation skills and ability to write evaluations, progress notes, and discharge summaries.
• Collaborates with disciplines to provide patient-centered care.
• Punctual, professional conduct and adherence to schedule.

Responsibilities

• Assess referred patients and determine rehabilitation needs.
• Develop and administer treatment programs with goals based on patient capacity.
• Document patient condition and progress; educate patients and families on therapy methods.
• Work with physicians and staff to ensure coordinated care.
