Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a PRN Physical Therapist to provide weekend rehabilitation services. Responsibilities include patient assessments, developing and implementing treatment plans, and educating patients and families.

Qualifications

• Must be a licensed Physical Therapist in good standing.
• Experience with long-term care and geriatric populations preferred but not required.
• Strong documentation and record-keeping abilities.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment plans with goals set by physician guidance.
• Set realistic goals and educate patients and families on therapy methods.
• Document patient condition, progress, and discharge needs.
• Supervise Physical Therapy Assistants and interdisciplinary staff.
• Collaborate with other disciplines to deliver patient-centered care.
• Maintain confidentiality and adhere to HIPAA and residents' rights.
• Punctuality and professionalism in scheduled hours.
