Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) for PRN weekend staffing. The PT will assess referred patients, design and supervise treatment plans, and educate patients and families on rehabilitation approaches.

Requirements include graduation from an accredited PT program, Tennessee license, and experience supervising assistants. Long-term care experience and thorough documentation are highly valued.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed to practice physical therapy in Tennessee.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care/geriatric settings preferred.
• Strong documentation and record-keeping skills.
• Ability to set goals and document progress.

Responsibilities

• Assess referred patients and provide direct PT care.
• Establish and administer treatment programs with goals.
• Document patient status and educate patients and families.
• Collaborate with all disciplines to deliver patient-centered care.
• Ensure confidentiality and HIPAA compliance.
