Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) on a PRN basis to cover weekend shifts at our facility in Tennessee. The PT will assess patients, develop treatment plans, and supervise assistants under physician direction, while maintaining thorough documentation and patient education.

Strong emphasis on geriatric care, patient-centered treatment, and collaboration with interdisciplinary teams.

Qualifications

• Graduate from accredited Physical Therapy program
• Valid PT license and state registration
• Experience supervising Physical Therapy Assistants
• Experience in long-term care/geriatric settings preferred
• Strong documentation and record-keeping
• Ability to perform evaluations and develop treatment plans
• Collaborates with other disciplines to provide patient-centered care
• Demonstrates professionalism and reliability
• Maintains confidentiality in accordance with HIPAA

Responsibilities

• Assess referred patients to determine rehabilitation needs.
• Provide direct patient care and implement treatment programs.
• Set realistic goals based on patient capacity and tolerance; adjust as needed.
• Document patient condition, progress, and outcomes; educate patients and families.
• Coordinate with Physicians and the care team to optimize recovery.
