Job summary

UnitedHealth Group is seeking a Licensed Physical Therapist to provide home health services. You will assess functional status, plan and implement treatment, and collaborate with the care team to improve patient outcomes in a home setting.

A flexible full-time weekend schedule is available (Friday-Sunday or Saturday-Monday) and a $10,000 sign-on bonus is offered for new hires. Responsibilities include evaluating patients, developing and adjusting plans of care, documenting progress, and ensuring

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire (preferred)

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment to the supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain max functioning.
• Instructs patient and family on plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care

Education

Current Physical Therapy licensure

CPR certification (preferred)
