Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports the patient's response to treatment and any changes in condition.
• Takes part in developing and carrying out the plan of care and goals.
• Completes patient evaluations and documents progress notes per policy.
