UnitedHealth Group is seeking a Physical Therapist for Home Health in New Hampshire. This full-time role offers a weekend schedule (Fri-Sun or Sat-Sun-Mon) with a $10,000 sign-on bonus.

You will assess patients, develop treatment plans, and deliver interventions to improve mobility, endurance, and well-being. Responsibilities include evaluating progress, coordinating with the care team, and documenting plans and outcomes while ensuring compliance with state regulations.

Qualifications

• Current Physical Therapy licensure in state of practice.
• CPR certification or ability to obtain within 90 days of hire.
• Valid driver's license and access to reliable transport.

Responsibilities

• Evaluates functional status and therapy needs, assists in plan of care.
• Provides PT services within scope and plan of care.
• Observes patient responses and reports changes to supervisors.
• Treats to relieve pain and restore function.
• Instructs patient and caregivers on plan of care and goals.
• Visits patients per Plan of Treatment and documents progress.
• Completes evaluations and develops PT plan within state guidelines.
• Reports outcomes and projected frequency of care.
