Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to perform assessments, develop treatment plans, and deliver interventions aimed at improving mobility and well-being. The role supports patients in their homes and collaborates with care teams to achieve outcomes.

Full-time position with weekend schedule options (Friday-Sunday or Saturday-Monday) and a $10,000 sign-on bonus for new hires, plus comprehensive benefits to support health and work-life balance.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification (preferred).

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports patient's response to treatment.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs patient and family regarding plan of care, limitations, and progress.
• Visits patients and completes progress notes.
• Completes all patient evaluations and develops PT plan of care.
• Reports outcomes of evaluation, goals, and frequency of care.
