Job summary

UnitedHealth Group's Caretenders division in Massachusetts seeks a licensed Physical Therapist for Home Health. You will assess patients' mobility, endurance, and pain, develop treatment plans, and deliver interventions to improve well-being while coordinating with physicians and nurses.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Informs patient and family about plan of care and progress toward goals.
• Visits patients, completes progress notes, and submits notes per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation and anticipated frequency of care.
