Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health to evaluate patient needs, determine treatment plans, and deliver interventions aimed at improving mobility and well-being. You will work with the healthcare team to implement care plans and monitor progress in a home health setting.

Responsibilities include evaluating functional status, developing the therapy plan, and educating patients and families on care goals while adhering to state guidelines and company policies.

Qualifications

• Current Physical Therapy licensure in the state of Washington.
• Ability to function in any home situation regardless of client characteristics.
• Access to dependable transportation or means to travel to patient homes.

Responsibilities

• Evaluates patient functional status and physical therapy needs.
• Assists physician in developing and updating the plan of care.
• Provides services within state practice scope and plan of care.
• Observes and reports patient responses to treatment to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family on plan of care, limitations, and progress.
