Job summary

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws governing physical therapy, per the plan of care, in coordination with the health care team.
• Observes and reports the patient’s response to treatment and any changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning per accepted standards.
• Instructs and informs the patient and family regarding plan of care, functional limitations, and progress toward goals.
• Visits patients per Plan of Treatment, completes progress notes, and submits them per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
