Job summary

Three Oaks Hospice, Inc. in Texas is seeking an On-Call Registered Nurse (RN) to provide skilled nursing care in patients' homes. You will coordinate with the interdisciplinary team and families to deliver compassionate, patient-centered care on evenings and weekends.

The role requires an RN license, CPR certification, and strong interpersonal skills; prior hospice or home health experience is preferred. A valid driver's license and reliable transport are required to visit patients.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency; and ensure coordination of care.
• Provide compassionate, patient-centered care on evenings and weekends as part of on-call coverage.
