Job summary

Canyon Ridge Hospital is seeking a compassionate Case Manager to join our behavioral health team on a per diem basis, Sundays and some Saturdays. You will provide case management, utilization review and psychotherapeutic services to psychiatric patients and their families within an interdisciplinary setting.

Qualifications

• Masters degree in Social Work required.
• Inpatient setting experience 2 years preferred.
• Written and oral communication skills, utilization management, case management, discharge planning, crisis intervention.
• Knowledge of acute adult psychological disorders.
• Assessing clients' needs and developing individualized care plans.
• Coordinating with healthcare providers, social services, and other stakeholders.
• Monitoring and evaluating clients' progress and adjusting care plans.
• Advanced principles of abnormal psychology as applied to adults, geriatrics, adolescents.

Responsibilities

• Provide quality case management, utilization review activities and psychotherapeutic services to psychiatric patients and families; serve as a member of interdisciplinary team.
• Ensure delivery of quality treatment within the treatment program.
