Job summary

Canyon Ridge Hospital is seeking a compassionate Case Manager to join our behavioral health team on a per diem basis, Sundays and some Saturdays. We emphasize patient-centered care and dual-diagnosis considerations, with a strong focus on collaborative treatment planning.

As part of an interdisciplinary team, you will provide case management, utilization review and psychotherapeutic services to psychiatric patients and their families, while maintaining quality and compliant discharge planning

Qualifications

• Masters degree in Social Work.
• 2 years of inpatient experience preferred.
• Strong communication skills; knowledge of utilization management and case management; time management; discharge planning and crisis intervention.
• Ability to assess clients' needs and develop individualized care plans.
• Coordination with healthcare providers, social services and other stakeholders.
• Monitoring and adjusting care plans based on progress.
• Advanced principles of abnormal psychology as applied to adults, geriatrics and adolescents.

Responsibilities

• Provide quality case management, utilization review activities and psychotherapeutic services to psychiatric patients and families.
• Serve as a member of interdisciplinary team supporting the organization’s treatment program and philosophy.
• Ensure the delivery of quality treatment to psychiatric patients and their families.
• Per Diem position on Sundays and some Saturdays.
