Job summary

Sierra Meadows Behavioral Health is seeking a Case Manager to oversee patient intake, progress notes, and discharge planning, coordinating with social, occupational, and educational resources as needed.

The role partners with the treatment team to guide clients through the continuum of care and ensure effective service delivery in a California-based setting. Schedule: Sun-Thu 2:00pm–10:30pm, full-time.

Qualifications

• Bachelor's Degree in Psychology, Social Work, or related field.
• Two years’ experience in behavioral health care setting.
• Strong leadership, communication, and organizational skills; able to work independently and as part of a team.
• Experience with diverse populations and equity initiatives within an organization.
• Proficiency in word processing and database or spreadsheet applications.

Responsibilities

• Admits new patients by completing intake and orientation.
• Evaluates patients’ needs for medical, psycho-social, and psychiatric treatment.
• Monitors cases, verifies attendance, documents progress, and coordinates services.
• Communicates progress by participating in weekly interdisciplinary meetings and sharing results.
• Prepares discharge plans and post-discharge resources.
