Job summary

Canyon Ridge Hospital in the Inland Empire is seeking a compassionate Case Manager to join our interdisciplinary behavioral health team. The role focuses on quality case management, utilization review, and psychotherapeutic services for psychiatric patients and families.

Per Diem Sundays and some Saturdays, with shift differentials for nights and weekends. Candidates should have a Master’s in Social Work, 2 years inpatient experience preferred, and strong skills in communication, discharge

Qualifications

• Masters degree in Social Work required.
• 2 years inpatient experience preferred.
• Written and verbal communication skills; knowledge of utilization management & case management.
• Discharge planning and crisis intervention skills.
• Knowledge of acute adult psychological disorders and crisis response.

Responsibilities

• Provide quality case management, utilization review activities and psychotherapeutic services to psychiatric patients and families.
• Serve as a member of an interdisciplinary team supporting the organization’s treatment program.
• Handle per diem scheduling with Sunday and some Saturday shifts.
• Coordinate care with healthcare providers, social services, and other stakeholders.
• Monitor and evaluate patient progress and adjust care plans as needed.
