Job summary

Connections Health Solutions in Tucson, AZ, seeks a Case Manager (Full Time, Back End Nights) to oversee care for youth and adults in crisis, aligning admission, treatment planning, and discharge processes with an interdisciplinary team.

You will document services, coordinate with providers, and ensure timely completion of mandated processes while contributing to a compassionate, patient-centered care environment.

Qualifications

• Associate’s degree in behavioral health or related field.
• At least 2 years of behavioral health experience.
• Arizona Level I Fingerprint Clearance Card.
• Company vaccination policy requires full vaccination or approved exemption.

Responsibilities

• Manage care for individuals in crisis, including youth and adults.
• Coordinate admissions intakes, treatment and discharge planning, and collateral collection.
• Collaborate with an Interdisciplinary Treatment Team to coordinate care and discharge planning.
• Maintain clinic records and ensure documentation is complete and accurate.
• Monitor involuntary processes and RBHA system knowledge for funding sources.
• Participate in supervision, education, and ongoing competency activities.
