Job summary

Promises Behavioral Health seeks a passionate Full-Time Case Manager to join our team in a dynamic behavioral health setting. The role requires weekend availability, strong case management skills, and LCDC or LCDC intern qualifications.

You will screen, assess, and coordinate services, develop individualized plans, and assist with housing and aftercare planning to support lasting recovery. Responsibilities include counseling, group facilitation, documentation, and collaboration with providers.

Qualifications

• Bachelor's degree or equivalent experience preferred.
• Knowledge of case management and behavioral health services.
• Experience with substance abuse and mental health services desired.
• Experience maintaining patient records and regulatory compliance.
• Ability to work within local, state, and federal requirements.

Responsibilities

• Plan, implement, and coordinate client services during residency.
• Deliver psychoeducational and motivational instruction in groups and individually.
• Perform patient screening, intake, and assessment.
• Develop and refine individualized service plans and referrals.
• Document in the patient record and maintain files per requirements.
• Coordinate transportation and assist with continuing care housing.
• Conduct toxicology screens as needed and participate in team meetings.
