Job summary

Valley Behavioral Health Incorporated in Salt Lake City, UT is seeking a Case Manager to provide psychosocial rehabilitative services per the care plan and Medicaid guidelines. You’ll support clients, document care, and coordinate services to promote stability and independence.

The role offers an evening shift (Tue-Sat, 4:00pm–12:00am) with a $2/hour differential, and a strong benefits package with growth opportunities through post-hire training and licensure support.

Qualifications

• High School diploma or equivalent required.
• Case Manager certification required.
• CPR certification (Post-hire) available.
• Valley de-escalation certification (Post-hire) available.

Responsibilities

• Provides psychosocial rehabilitative services as indicated by the Medicaid manual.
• Supports clients in following individualized care plans and identifying goals.
• Completes documentation on services within Valley’s standards.
• Meets productivity expectations as assigned by supervisor.
• Distributes medications as necessary.
• Transports clients to appointments as necessary.
