Job summary

Cadence is hiring a part-time, remote Clinical Navigator (LPN/LVN) to provide virtual chronic care support for Medicare patients. You will conduct outreach, coaching, and care coordination, addressing barriers like access to resources and transportation.

Document interactions and collaborate with clinicians to ensure patient engagement and timely follow-up. Requires an active compact multi-state LPN/LVN license, 5+ years of experience, and remote-work readiness; Spanish preferred.

Qualifications

• Active compact multi-state LPN/LVN license in the state of residence.
• At least 5 years of clinical experience as an LPN.
• Remote work readiness with stable high-speed WiFi.
• Active CA state LVN license required.
• Spanish fluency preferred.
• Experience providing patient education and support for chronic conditions preferred.
• Familiar with AI-assisted clinical tools and can discuss AI support vs. human judgment.

Responsibilities

• Deliver proactive, patient-centered outreach and coaching to Medicare patients.
• Support care plans with education, motivation, and SMART goals.
• Assess health status, readiness to change, and barriers to care.
• Educate on disease management, adherence, and prevention; document interactions.
• Monitor progress and outcomes; coordinate with clinical teams and resources.
• Document interactions in Cadence Platform; use tools like Google Workspace and Slack.
