Job summary

Cadence is seeking a part-time Clinical Navigator (LPN/LVN) to deliver virtual chronic care support for Medicare patients. You will conduct proactive outreach, coaching, and care coordination to close gaps in care while documenting progress in Cadence platform and collaborating with the clinical team.

The role requires a current LPN/LVN license, 5+ years of nursing experience, and strong communication skills.

Qualifications

• Active LPN/LVN license in current state is required.
• Minimum 5 years of clinical nursing experience as an LPN/LVN.

Responsibilities

• Provide outbound patient outreach and one-on-one coaching for chronic conditions.
• Educate and coach patients on personalized care plans and self-management strategies.
• Assess health status, readiness to change, and barriers to care.
• Educate on disease management, adherence, symptom recognition, and prevention.
• Monitor progress and health outcomes with remote monitoring tools.
• Document interactions and coordinate with clinicians through Cadence platform.
• Collaborate with care teams using Cadence, Google Workspace and Slack.
