Qualifications

• Licensed Practical Nurse with multi-state license required.
• Minimum of 5 years clinical experience as LPN/LVN.
• Remote-work readiness and stable high-speed internet.
• Active California LVN license required; Spanish a plus.

Responsibilities

• Outreach to patients managing chronic conditions via phone.
• Provide coaching and education to support personalized care plans.

• Collaborate with clinicians and support teams to coordinate care.
