Job summary

Cadence is hiring a Clinical Navigator (LPN) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This part-time, remote role requires evaluating patients, coaching, and coordinating care within Cadence policies.

You will document interactions, manage barriers to care, and collaborate with the clinical team while using Cadence platforms and AI tools. The position is remote in the United States.

Qualifications

• Active compact multi-state LPN/LVN license in the state you reside.
• At least 5 years of clinical experience as an LPN.
• Remote work readiness with stable high-speed WiFi.
• Active CA state LVN license is required.
• Spanish language proficiency preferred.
• Experience educating patients with chronic conditions preferred.
• Familiarity with AI-assisted clinical tools and discussing AI-supported care and human judgment.

Responsibilities

• Outreach by phone and one-on-one coaching to patients with chronic conditions.
• Help patients execute personalized care plans with education and behavior change coaching.
• Assess health status, lifestyle, and readiness to change.
• Educate on disease management, medication adherence, and prevention under clinical guidance.
• Monitor patient progress and health outcomes via remote tools.
• Document interactions and connect patients to resources as needed.
• Collaborate with clinicians using Cadence Platform, Google Workspace and Slack; offer workflow improvements.
