Job summary

Personal Healthcare Management LLC is seeking an LPN for overnight, full-time shifts in Downsville, NY. The role features 12-hour night shifts from 7:00pm to 7:00am, three days a week, with every other weekend. A sign-on bonus of $5,000 is available for FT employees over a two-year period.

Qualifications

• Minimum 1 year of nursing experience.
• LPN license/registration required.

Responsibilities

• Provides safe resident care and administers treatments according to standards.
• Communicates resident information and plan of care during transfers and shift reports.
• Administers medications following the five rights and documents accordingly.
• Performs treatments using sterile or clean technique.
• Completes care activities within scheduled shifts and cooperates with reassignments.

Functions

• Provides safe resident care, demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies. Maintains awareness of new updates standards and incorporates them into his/her nursing practice throughout the continuum of care.
• Participates in the improvement process of quality resident care.
• Incorporates age-specific needs, provision of care, procedures, and treatments.
• Communicates pertinent resident information and plan of care before breaks, during transfer, and at shift report.
• Accurately administers medications according to the five rights of medication administration and documentation of same.
• Accurately performs treatments utilizing sterile or clean technique.
• Accurately administers external feedings.
• Completes resident care-related activities within scheduled shift time periods.
• Cooperates with reassignments made in the interest of resident care.
• Reports resident care problems and possible solutions to the appropriate nursing management personnel.
