Job summary

Personal Healthcare Management LLC seeks an LPN for the overnight shift at our Oneonta location. The role is full-time with 12-hour night shifts (7:00 pm to 7:00 am) and every other weekend, three days a week.

Starting pay is $35.00–$35.50 per hour with evening/night differentials up to $36.75, plus a $5,000 sign-on bonus paid over two years. Benefits include 401(k), health, dental, vision, and PTO.

Qualifications

• LPN license/registration required.
• 1 year of experience preferred.
• Must be able to perform nursing duties within scope of practice.

Responsibilities

• Provide safe resident care and perform assigned treatments per standards.
• Document care and communicate plans during handoffs.
• Administer medications according to five rights and document accurately.
• Support continuous quality improvement in resident care.
• Collaborate with nursing leadership during shift changes and transfers.

Functions

• Provides safe resident care, demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies. Maintains awareness of new updates standards and incorporates them into his/her nursing practice throughout the continuum of care.
• Participates in the improvement process of quality resident care.
• Incorporates age-specific needs, provision of care, procedures, and treatments.
• Communicates pertinent resident information and plan of care before breaks, during transfer, and at shift report.
• Accurately administers medications according to the five rights of medication administration and documentation of same.
• Accurately performs treatments utilizing sterile or clean technique.
• Accurately administers external feedings.
• Completes resident care-related activities within scheduled shift time periods.
• Cooperates with reassignments made in the interest of resident care.
• Reports resident care problems and possible solutions to the appropriate nursing management personnel.
